Cardiovascular disease has become one of the major chronic diseases in Taiwan because of aging population, changing of life style and dietary habits. Since 2004, cardiovascular disease has been maintained the second leading cause of death in Taiwan after malignant tumors. In 2013, 17694 people died from heart disease, accounting for 11.5% of deaths. Among them, acute coronary syndrome is the most serious illness, including ST segment elevation myocardial infarction, non-ST elevation myocardial infarction and unstable angina.
Coronary artery disease has certain mortality rate. Clinical diagnosis should be based on the character of chest pain, electrocardiogram, blood biochemical cardiac biomarkers and other related tests to increase the sensitivity and eliminate the possibility of other disease. However, there is still some atypical performance of coronary artery disease. Image study could help us make diagnosis.
For ST elevation myocardial infarction (STEMI), time is life. The sooner the rescue is the more benefit for the patient. Kaohsiung Veterans General Hospital (KSVGH) promoted the hospital rescue process in 2006. KSVGH cooperated with Fire Bureau and Department of Health, Kaohsiung City government, to establish Pre-hospital 12-lead Electrocardiogram system. After calling 119, ambulance will performed 12-lead Electrocardiogram by Ultra-rapid electrocardiogram device while arrival. It takes 30 seconds to examination and transmission. 30 seconds save a lifetime. The electrocardiogram can be transmitted to cardiologist’s mobile phone immediately and activated the catheterization to rescue the patient. Numbers of cities in Taiwan have established this life-saving system now.
Three categories of acute coronary syndrome treatment are different. Patients with ST segment elevation myocardial infarction needs emergency cardiac catheterization within 12 hours. In patients with non-ST elevation myocardial infarction or unstable angina, it determined whether cardiac catheterization or medication treatment due to patient’s condition. No mater cardiac catheterization or not, long-term medication therapy can improve symptoms and reduce mortality. Patients must cooperate with doctors for regular medication treatment.
After the horrible experience of acute coronary heart attack, many patients and their family have many doubts about whether they can keep daily activities or not, even continue to exercise. They are worried about if the heart event will be triggered by these events. Cardiac rehabilitation can be started after treatment of acute coronary heart disease immediately under stable condition. Therefore, how to safely and effectively exercise to assist the recovery of body functions will be an important task of cardiac rehabilitation.
Some factors have been proved with relation to coronary artery disease, including hyperlipidemia, hypertension, diabetes mellitus, impaired renal function, metabolic syndrome, overweight, obesity, unbalanced diet, lack of exercise, smoking, and stress. Therefore, healthy life style is very important for the prevention of coronary artery disease, including enough sleep, avoid fatigue, no smoking, less drinking, measure blood pressure regularly, maintain ideal body weight, keep warm, maintain a good mood.
